Bell 407 – Missed Tool Caused Main Rotor Damage

Bell 407 main rotor damage from tool left on another helicopter's rotorhead

Maintenance was being performed on a rotor head of a nearby helicopter. The pilot and maintenance person did not do a thorough enough check on prelfight and started the helicopter up with a tool left on the rotor head… The tool departed the rotor head and went through a nearby helicopter's blade and hit a different helicopter on the the fuselage.

Always do a thorough preflight after/during Mx and account for all tools…
